Here is pretty much like the last picture posted plus a little cleanup.
But you see the Crusty made cover protrudes upward beyond what will be the lower surface of the to be made cover.Adjustments to pipe are a quick fix with the right tools.
Extra care is a good thing as chucks tile don't work well in a sewer line.
Next a form for the new cover.
Concrete isn't too strong when it's thin so some steel might help. I'm hoping it'll help ENOUGH.
What to do with a little leftover concrete- I've been meaning to do this for along time. It's a drain that coveys water form the patio into the yard. actually at one time it sort of did the opposite but that a long story. It gets covered with a shallow layer for granite but this should help the water get away and not make a big hollow space under the patio floor.
